Please download and complete a New Patient form before coming to your first dental visit.  You can e-mail, FAX, or bring a printed version of the form to our office.  This will facilitate processing your medical, dental and insurance information prior to your appointment.

A special patient referral form is also available for download (ReferralGP-BC.pdf) for other health professional who would like to refer their patients for oral disease diagnosis and dental treatments.
